New Patients - Cash, check, credit card, or insurance is required for all first visits. Please bring your insurance card with you to your appointment.
Please remember, your dental insurance is your responsibility: it is a contract between you and your insurance company, but we can help!
As a courtesy, we will file your insurance claim. Regardless of what we might estimate as your dental benefit in dollars, we must stress the fact it is an estimate based on the best information we have at hand. You, the patient, are responsible for the treatment fee regardless of what your insurance does or does not pay.
- Your deductible should be paid at your first visit. Any out-of-pocket portion not paid by insurance should be paid at the time the service is rendered.
- We will only file primary insurance claims, not secondary.
- If you provide incorrect insurance information and a claim is denied, you will be responsible for refiling the claim.
- Should your claims be denied, all payment of remaining balance due becomes your responsibility.
